How do you understand a company's real problems?

We analyze where information enters, who uses it, where it gets copied, which activities block the work, and which errors keep repeating. We interview the people involved, observe the tools already in use, and reconstruct the real process, exceptions included. The problem that needs solving is often different from the one perceived at first.
