How do you reduce a chatbot's hallucinations?

Hallucinations can't be eliminated completely, but they can be reduced by combining controlled sources, retrieval, precise instructions, confidence thresholds, citations, validation, and fallbacks. For critical tasks, deterministic rules and human approval are added. Normal, ambiguous, and adversarial questions are also tested before launch.
